1. Acceptance of Terms

By accessing or using the services of Carolinas Depression Clinic, you agree to be bound by these Terms of Service. If you do not agree, please do not use our services.

2. Services Provided

Carolinas Depression Clinic provides licensed mental health treatment services, which may include individual, couples, family, and group treatment delivered in person or via telehealth (as permitted by state and federal law).

3. Eligibility

You must be at least 18 years old to receive services unless a parent or legal guardian provides consent for minors under applicable laws.

4. Confidentiality

We follow HIPAA guidelines and applicable federal and state privacy laws to maintain the confidentiality of your personal health information. Exceptions include:

  • Risk of harm to self or others
  • Suspected abuse or neglect of a child, elder, or dependent adult
  • Court orders or legal requirements

Please refer to our Notice of Privacy Practices for detailed information.

5. Telehealth Services

If services are provided via telehealth:

  • You must be physically located in a state where your provider is licensed.
  • A stable internet connection and private space are required.
  • Sessions may not be recorded without written consent from both parties.

6. Appointments & Cancellations

  • Sessions are by appointment only.
  • Please provide at least 24 hours’ notice for cancellations. Late cancellations or no-shows may incur a fee as specified in our Fee Agreement.

7. Fees & Payment

  • Fees for services will be discussed before your first session.
  • Payment is due at the time of service unless otherwise agreed upon.
  • We accept cash, financing plans (CareCredit & Advance Care Card) and insurance plans, if applicable (Blue Cross Blue Shield NC, Cigna, Tricare, Medicare).

8. Emergencies

Carolinas Depression Clinic is not a crisis service. If you are experiencing a life-threatening emergency, call 911 or go to the nearest emergency room. For mental health crises, contact the 988 Suicide & Crisis Lifeline.

9. Termination of Services

Treatment services may be terminated if:

  • You repeatedly miss or cancel sessions without notice.
  • You are not actively participating in treatment.
  • Your needs exceed the provider’s scope of practice or expertise.

Referrals will be provided if possible and appropriate.

10. Code of Conduct

Clients must behave respectfully toward staff and therapists. Abusive, harassing, or threatening behavior may result in termination of services.

11. Consent to Treatment

By engaging in treatment, you are giving informed consent to receive treatment from a licensed mental health professional in accordance with professional and ethical standards.

12. Modifications to Terms

Carolinas Depression Clinic may revise these Terms of Service at any time. Clients will be notified of any material changes. Continued use of services after changes constitutes acceptance.

13. Governing Law

These Terms shall be governed by the laws of the State of North Carolina, without regard to its conflict of law principles.
